​Please take a moment and download your personal history statement and waivers (steps 1 and 2). Save the forms on your computer and open them with Adobe Acrobat Reader DC . This will allow you to save your work as you complete the documents and sign documents with Adobe Signature. You can also sign the documents by hand and scan them.

When you have completed filling out your forms upload all completed work and documents, in our Document Upload Portal (step 3).

Waivers / Releases

As part of each background investigation, applicants are required to review and sign the attached waivers and advisements. If you have any questions about any of the documents, please ask your background investigator for an explanation.

Personal History Statement

Your personal history statement is an extensive document that will take some time to complete. Please allow for at least one week to complete the PHS and ensure that all fields are filled in with data or "N/A" if they are not applicable.
